School / Group List – This form is a necessary component of submitting a scoring order to CTC. On this form, you will document and organize the tested students into groups. A group is defined as all the students tested in the same grade level, testing level, and class. An order from a large school will typically have many groups. Simply fill out the grade, teacher name, and testing level columns of each group on the list before submitting an order.

CTC Order Form – This is the generic form used for ordering products from CTC. It is necessary to fill this out when ordering the CAT, Insight, CTCS or any other product from CTC.
